Appended Terms and Conditions
for Contact Lenses and Prescription Eyewear: In
addition to the Terms and Conditions of Purchase,
every user that purchases any prescription product (i.e.
contact lenses, prescription eyewear, etc.) is bound by
the contract of the Appended Terms and
Conditions for Contact Lenses and Prescription Eyewear.

If you use the EZContactsUSA website, you are consenting to agree to the following terms and conditions. The terms and conditions can be altered at anytime without notification and your continued use of the website indicates your agreeance to these amendments.
EZContactsUSA is owned and operated by EZContactsUSA and no content can be copied or reproduced without the written consent of EZContactsUSA.
EZContactsUSA is a website intended for shoppers that want to browse and purchase EZContactsUSA products and merchandise. No malicious or unlawful activity is permitted. No activity that will negatively affect the performance of the website is permitted.
EZContactsUSA is not responsible for any errors, including errors in the description or pricing of products. However, EZContactsUSA reserves the right to correct any errors at any time without notice.
Terms and Conditions of Purchase
This contract applies to all purchases made through EZContactsUSA. Purchasing is defined as placing an order with EZContactsUSA via phone, email, website, fax, or other means.
Once an order is placed, it cannot be altered. EZContactsUSA aims to meet the specifications of your order to the best of its ability. However, there is no guarantee that there will be no errors.
Once a purchase is submitted, EZContactsUSA will provide you an e-mail confirmation of the order. This e-mail confirmation is intended as a reference to your purchase and does not in anyway bind EZContactsUSA to sending you the ordered product.
EZ Contacts does not guarantee stock availability. In the case that a product is not in stock, EZContactsUSA will contact you with 24 hours with a choice of a replacement or a refund. If EZContactsUSA receives no response, the order will be cancelled and your money refunded.
Merchandise will be delivered to the address indicated in an order. EZContactsUSA is not responsible for products sent to a wrong address if an incorrect address was provided.
Once you receive your purchased order, it is your responsibility to inspect the product immediately. If any merchandise does not match the description or is defective or missing, do not use the product and contact EZContactsUSA immediately. EZContactsUSA will rectify the matter accordingly.
EZContactsUSA will pay for the cost of postage for defective goods. However, you are liable for the merchandise until it is received by EZContactsUSA.
Appended Terms and Conditions for Contact Lenses and Prescription Eyewear:
This contract applies to all purchases of prescription products. Purchasing is defined as placing an order with EZContactsUSA via phone, email, website, fax, or other means. Prescription purchases are defined as products that are prescribed by a licensed optometrist such as prescription eyewear or contacts.
To purchase prescription products, you must have a prescription from a registered optometrist. By submitting a prescription purchase, you are guaranteeing that you possess a written prescription from a registered optometrist with prescription being recent and in accordance with your optometrist's prescribed regimen.
We reserve the right to contact your optometrist to verify your prescription or to have you fax a copy of your original prescription to EZContactsUSA.
The prescription of all prescription products is included in the product details. It is your responsibility to verify that the prescription of the product matches the prescription given to you by your optometrist before purchasing and before usage. In the case that the prescription does not match the prescription you submitted in your order, do not use the merchandise and contact EZContactsUSA immediately.
If purchasing a prescription product, you must also review and consent to the Terms and Conditions of Purchase contract.
